New Patients – In order to register with the practice you should bring your NHS medical card. Don’t worry if you can’t find it, but if you have it, please bring it.
You will be asked to fill in a registration form and a new patient questionnaire. All new patients are asked to see our Practice Sister who will discuss your medical history and medication, and will record you height, weight and blood pressure as a base-line recording.
Please bring a urine sample when attending this check. Please remember it may take you several weeks for your case notes to be transferred from your previous doctor.
